Focuses on the need to identify children and families at optimal points in the development of the child and life of the family. Prepare families to support their children as they become Ready for School; Ready for Life. Expansion of Healthy Families and providing Part C screening for all at-risk children. Provide annual training for outreach point organization on screening and referral with monitoring and modification of screenings. Work in conjunction with the Ready Services committee to establish a transition and school readiness plan for rising Kindergarten students.
